A century before Joe Biden took office, a U.S. president was so mentally incapacitated that the first lady became de facto commander in chief.

Due to lack of political will and no clear-cut guidance from the Constitution, no one — not even the vice president — intervened as Edith Wilson essentially took over for President Woodrow Wilson.
